Telehealth Informed Consent
Before your first visit, please read the following.

This Telehealth Informed Consent (“Consent”) describes the nature of the telehealth services offered through Pephaus LLC (“PepHaus,” “we,” “our,” or “us”), and the benefits, risks, and alternatives of receiving care in this manner. By completing a PepHaus intake and requesting a visit, you acknowledge that you have read, understood, and agreed to the terms set out here.
1. What Telehealth Means at PepHaus
Telehealth is the delivery of health-care services using secure electronic communication between a patient and a licensed health-care provider who are not in the same physical location. PepHaus offers asynchronous (“store-and-forward”) telehealth: you complete a written medical intake through our Site, and a U.S.-licensed provider credentialed in your state reviews your information and communicates with you through the PepHaus patient portal and by secure email.
Live (synchronous) video or telephone consultations may be offered at the provider’s discretion when clinically appropriate.
2. Benefits of Telehealth
- Convenient access to a licensed provider without in-person travel;
- Greater privacy and discretion than a traditional pharmacy visit;
- Faster turnaround for medically appropriate prescriptions;
- Ongoing written communication with a care team you can reference later.
3. Risks and Limitations of Telehealth
Telehealth is not appropriate for every condition or every patient. By consenting to telehealth, you acknowledge the following risks and limitations:
- Your provider cannot perform a physical examination. Some conditions cannot be safely diagnosed or treated without one.
- Clinical decisions are based on the information you provide. Missing, inaccurate, or incomplete information may lead to a declined visit or an incorrect assessment.
- Electronic transmissions can, on rare occasions, be disrupted, delayed, or intercepted despite reasonable safeguards. PepHaus uses HIPAA-compliant systems to minimize this risk but cannot guarantee that transmission will always be secure.
- Your provider may determine that telehealth is not appropriate for your situation and may refer you to in-person care, a different specialist, or an emergency facility.
- Technology may fail. If you do not receive a response within the expected time frame, contact hello@thepephaus.com.
4. Not for Emergencies
PepHaus is not an emergency service and is not a substitute for emergency medical care. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department. If you are experiencing suicidal thoughts or a mental-health crisis, call or text 988 (the U.S. Suicide and Crisis Lifeline). Do not rely on the PepHaus patient portal, email, or messaging for urgent or time-critical symptoms.
5. Alternatives to Telehealth
You always have the right to seek care in person. Alternatives to PepHaus telehealth include, without limitation, an in-person visit to your primary-care provider, an in-person specialist visit, a visit to an urgent-care clinic, or an emergency-room visit. Your provider may recommend one of these alternatives if it is clinically appropriate.
6. The Provider-Patient Relationship
A provider-patient relationship is established only after a U.S.-licensed provider reviews your intake and accepts you as a patient. Submitting an intake does not, by itself, create a provider-patient relationship or guarantee that a prescription will be issued. The provider exercises independent medical judgment and may decline to prescribe any medication for any clinical reason.
7. Compounded Medications
PepHaus may arrange for certain medications, including compounded tirzepatide, to be prepared by a state-licensed 503A compounding pharmacy pursuant to a valid prescription. Compounded medications are not FDA-approved, have not been evaluated by the FDA for safety or effectiveness, and do not undergo the FDA premarket-approval process. Compounded medications are prescribed only when a provider determines that a commercially available FDA-approved drug is not clinically appropriate for the individual patient.
8. Privacy and Records
Your health information is treated in accordance with our HIPAA Notice of Privacy Practices and our Privacy Policy. You have the right to inspect and receive a copy of your medical record, to request corrections to it, and to understand how your protected health information is used and disclosed.
9. Your Rights
You have the following rights with respect to PepHaus telehealth:
- To withhold or withdraw consent to telehealth at any time, without affecting your right to future in-person care;
- To receive the same ethical and legal standards of care as you would in an in-person visit;
- To request an in-person referral when clinically appropriate;
- To access, copy, and request correction of your medical record;
- To confidentiality of your health information, subject to the exceptions set forth in our HIPAA Notice.
10. Financial Considerations
PepHaus is a cash-pay service. We do not bill insurance. The total price you see before you pay is the total price you pay for that cycle. 11. State-Specific Provisions
Your provider must be licensed and credentialed in the state in which you are physically located at the time of your visit. If you move or travel to another state, your eligibility to continue receiving care through PepHaus may change. You are responsible for informing your care team of any change in your state of residence.
12. Acknowledgement
By accepting this Consent, you confirm that:
- You have read and understood the nature, benefits, and risks of asynchronous telehealth;
- You understand that PepHaus is not an emergency service;
- You understand that compounded medications are not FDA-approved;
- You will provide complete and truthful information to your provider;
- You may withdraw this Consent at any time by emailing hello@thepephaus.com.
